If you wish to make a request for treatment, please send us an e-mail with short information about your diagnosis and the medical services you are interested in. Our team of Costumer Service Managers will take care of your request and guide you through the process.

Please send copies of medical documentation in English (i.e. medical reports, imaging in DICOM format) by ordinary mail or courier, or if you prefer, and at your own responsibility, via e-mail.

Storage of personal data
According to the Personal Data Act, Stockholm Care (SC) is obliged to inform you that we may store personal data you submit to us. SC will store personal data (eg name, address, date of birth and diagnosis) in a database for future reference. The data will be available to staff of SC and contracted caregivers. The information may be used for (but not limited to) process development, obtaining of statistics, possible future reconnection and to secure traceability. If you have sent us personal data you may at any time send a written request to be informed about the content of your file. You may also request in writing, to have information corrected, partly removed or fully removed from the SC records.
